Last name first. Use initials for first and middle name with no commas in between. Place a comma between multiple authors. End with a period. Many web sites do not have an author listed, so skip to the title of the web page if no author is given.

Title of the specific item cited: Information for physical therapy consumers.
Title and subtitle, if any, are separated by a colon. Capitalize only the first word of the title and any proper nouns. If no title is provided, use the name of the organization responsible for the site. End with a period.  

Name of the website: The American Physical Therapy Association.
Name of the website. End with a period. 

URL: http://www.apta.org/Content/NavigationMenu/Consumers/consumer1.htm.
Give the web site address. End with a period.

Published Date: Published November 2007.
If available, list the published date of the website proceeded by the word Published. End with a period.

Updated Date: (If applicable)
If available, list the updated date of the web site preceded by the word Updated. End with a period.

Accessed Date: Accessed September 17, 2008.
Always provide the date you accessed (viewed ) the page preceded by the word Accessed. End citation with a period.
